School Nurse

School nursing is a specialized practice of professional nursing that advances the well-being, academic success and life-long achievement and health of students.  To that end, school nurses facilitate positive student responses to normal development; promote health and safety, including a healthy environment; intervene with actual and potential health problems; provide case management services; and actively collaborate with others to build student and family capacity for adaptation, self-management, self advocacy, and learning.  (School Nurse Definition: National Association of School Nurses, 2010)

Immunizations:  Please see attached chart outlining the State of Texas mandated immunization requirements: http://www.dshs.state.tx.us/immunize/docs/school/6-14_2010-2011.pdf.
Students must be fully compliant with state requirements to attend/enroll in classes in Carroll Independent School District. You must show documentation of immunization compliance before August 24, 2010.  There have been added immunizations to those students entering Kindergarten and 7th grade.  Please contact your school nurse if you have any questions regarding the immunization requirements.


Clinic Policies

Medications: All medications, prescription and over the counter must be stored in the Nurses Clinic according to the Carroll Independent School District Policies and Procedures.  Along with the medication, a Medication Administration form must be filled out and signed by the parent or guardian.  All prescription medication, if ordered for more than 10 days, must be in the original container and have a Medication Administration form signed by the prescribing physician containing the students name, dose, time, and administration route.  Students will only receive medications that have been provided by the parent or guardian.
Exclusions:  It is the goal of Carroll ISD to provide an environment that is conducive to learning for all students and staff.  Some communicable diseases or conditions may warrant exclusion from school to avoid exposing other students.  If your student exhibits any of the following symptoms, please keep your student home until the child is free of the symptoms, has been satisfactorily treated or submits a signed physicians statement that he/she is not contagious:

•Temperature of 100 degrees or above
•Vomiting and/or diarrhea
•Persistent cough
•Rash
•Open, itchy or draining lesions
•Red, itchy, draining eyes
•Presence of live lice or nits in the hair
•Suffering from a reportable disease, as defined by the Texas Board of Health
•Undiagnosed scaly patched on the body or scalp

Health Screenings

The Hearing and Vision Screening Program, Chapter 36 of the Texas Health and Safety Code requires all children enrolled for the first time in any public or private school be screened with an examination for possible hearing and vision problems.  The requirements apply each year to newly enrolled students and all students in Pre-Kindergarten, Kindergarten, First, Third, Fifth and Seventh Grade.  Once the initial screen is performed, those students who failed the initial exam will be re-screened 2 weeks later.  If the student fails the second screen, a referral letter will be sent home with the student by your school nurse. 

Spinal Screen (All students in Fifth and Eighth grade)
Acanthosis Nigricans:  All students in First, Third, Fifth and Seventh grade.

Vaccination Clinics

Carroll ISD will be hosting a Flu vaccine clinic in the Fall, please check back for updates. 

Tarrant County Health Department will return in the fall of 2010 and again in the spring of 2011 with our Immunization Outreach program.  Those students in grades 6 through 12 will have the opportunity to pre- register for state required vaccines to be given at their school campus.  Prices are $14 for 1 vaccine and $20 for 2 or more vaccines, all students are eligible.  Please watch for further updates through this site and your student’s school web site. 

 

 

Forms

Updated District Immunization Requirements Released

According to the Texas Attorney General, under the Texas Administrative Code section 97.66, the law requires that students be fully immunized against specific diseases before enrollment. The minimum state requirements are listed below:

Vaccine: Required Doses:
Diphtheria, Tetanus, Pertussis
(Dtp, DTaP, DT,Td,Tdap)
Five doses of any combination DTaP/DTP unless 4th dose was given on or after 4th birthday in which four would be sufficient.
Students 7 years of age and older:
  • Three doses of any combination DTP/DTaP/TD vaccine (one given after age 4)
  • One dose of T containing vaccine required ten years after last dose of DTP/DTaP/DT
  • Tdap upon entry into 7th grade if it has been 5 years since receiving a tetanus-diphtheria containing vaccine. When 5 years elapsed since T then Tdap due.

Polio
(IVP)

Four doses unless the 3rd dose was on or
after the 4th birthday in which three doses
would then be sufficient.
No polio after age 18.
Measles, Mumps, Rubella
(MMR)
Kindergarten – two doses MMR after 1 year
Grades 1 – 12 - two doses of a measles
containing vaccine with the first dose on or
after the 1st birthday; one dose of Mumps
vaccine and one dose of Rubella vaccine
Hepatitis B Three doses are required for all students
Per-K – 12 grades
Varicella Kindergarten and 7th grades – Two doses on
or after 1st birthday (or written statement
from parent, school nurse, or physician of
history of varicella is acceptable in lieu of
either dose)
Grades 1-6 and 8-12 require one dose.
(Two doses if vaccine given at 13 years of
age or older)
Meningococcal Vaccine All students entering 7th grade beginning in
August 2009 must have one dose.

Please note that students under age five who are enrolling in the district must have the specified number of doses for each vaccine:

DTP/DTaP/DT

4 doses
Hib 1 dose given at or after 15 months or a
series with a booster given on or
after 12 months of age.
Polio 3 doses
Measles, Mumps, Rubella 2 doses
Varicella 2 doses (or written statement from parent,
school nurse, or physician of history of
disease is acceptable in lieu of either dose)
Hepatitis B 3 doses
Hepatitis A Pre-K and Kindergarten
2 doses with first on or after 1st birthday
